Description: The form used by Urban students to indicate their preferences for their schedule.

The students indicate their ordered preferences within each scheduled period, then meet with a faculty advisor who also indicates hir own preference for the student.

Since Urban doesn't divide students into grades, courses are not restricted to students from a specific grade, but will include students of various ages.
